NARRATIVE SUMMARY

HMM-364 flew  1,280.8 hours completing 1,378 tasks for 4,940 sorties, lifted 254 tons of cargo, carried 11,440  troops/passengers and  flew 189 medevac  missions with 90 emergency,  87 priority and 285 routine evacuees.   There were 27 enemy fire  incidents directed  at squadron  aircraft which resulted  in only two  aircraft receiving hits.   The average aircraft availability  was 10 and  aircrew  availability was 14.   The squadron awarded 1  Distinguished Flying Cross, 2  Single Mission Air  Medals,  1 Purple Heart,  21 Strike  Flight  Award  Medals  and  29  Aircrew Wings.

PART III
SEQUENTIAL LISTING OF SIGNIFICANR EVENTS

4 July - Aircraft  153355  damaged  three  aft  blades  in an unsuitable zone on a recon insert.

10 July - Aircraft  154815  damaged  one  blade  on a routine recon extract.  The blade struck a small tree on the left side of the aircraft.  No enemy fire  received.

17 July - Aircraft 153355 damaged  blades on an  emergency recon  extract.  The recon  team was in  contact with  the enemy and the aircraft, also under fire, was hovering over the zone when the blades contacted the trees.

24 July - Aircraft 154814, on a troop lift,  lost power in a confined mountain top zone.  In its uncontrollable state the aircraft  hit the ground and then  numerous trees in  the zone.  Three aft blades, three forward blades and  the forward  rotor head  were damaged.   The aircraft  took  fire pervious  to  the landing.   Aircraft 153342  damaged  three forward blades on an emergency recon extract when  the aircraft went IFR in the zone.  This aircraft was also taking fire.  Aircraft 154827 damaged three aft blades on a routine recon insert into an unsuitable zone.

27 July - Aircraft  154027  damaged  three  aft  blades  and  took  two hits in the cockpit  while conducting a  ladder extract  of a recon  team in contact  with  the enemy.  Aircraft  153354 damaged one  aft blade on an emergency  recon extract while hovering against sloping terrain.

28 July - High day for the month, HMM-364 flew 50 flights for 69.5  total hours, 274 sorties, 73 tasks,  carrying 627 troops/passengers and 16.5  tons of cargo and flew two medevac missions for three evacuees.
